Sports Physicals 2026

IACH appointments for school and sports physicals are now available for May and going through August. These appointments are reserved for youth enrolled to a primary care manager at IACH.
Your child may need a school or sports physical if they meet any of the following:
• Ages 8 or younger and entering a Kansas school for the first time
• Participating in school-organized sports (middle or high school)
• Enrolled in Fort Riley’s Child & Youth Services (CYS) daycare or youth sports programs
Appointment slots are limited, and demand increases as the school year approaches. Call (785) 239-DOCS (3627) to secure your child’s physical before their program deadline.
Before your appointment, download the correct school or sports physical form from your child’s school district or organization. Parents/guardians must complete all applicable sections before the visit to avoid delays.
Special Note for CYS Participants:
If your child already had a well-child visit within the past 12 months, a new appointment is not required to complete the CYS physical. Instead, submit your completed CYS forms by one of the following methods:
• Drop off at the Patient-Centered Medical Home front desk
• Upload via the MHS GENESIS Patient Portal
• Call and leave a message requesting the paperwork be completed
Please allow up to 72 business hours for processing.
