Access Your Medical Records

At Irwin Army Community Hospital, we make it easy for you to access your medical records. Here’s how:

MHS Genesis Patient Portal

We encourage you to use the MHS Genesis Patient Portal to access your medical records. To register, you’ll need a DoD Self-Service Logon (Level 2) account.

What’s Available on the Portal?

  • Lab Results
  • Radiology Reports
  • Clinical Notes & Documents
  • Immunization Records
  • Medication Lists
  • Appointment Lists
  • COVID-19 Results

Using the portal is the fastest way to access your records.

Requesting Medical Records

If you need a copy of your medical records, we use the DoD Secure Access File Exchange (DoD SAFE), a secure and encrypted platform, to deliver them.

How to Request Records:

  1. Complete the DD Form 2870.
  2. Email the form to: dha.riley.irwin-ach.mbx.iach-ft-riley-correspondence-roi@health.mil.
Important Notes:
  • Cost: Your first copy is free. Duplicate copies cost $18.97, payable upon request.
  • Timeline: Routine requests take up to 30 working days.
  • Notifications: You’ll receive a text and email with a DoD SAFE link to retrieve your records. Check your junk folder if you don’t see it. Link is active for only 7 days. If the link expired, you may be asked to submit a new request.
