Patient Records

MHS Genesis Patient Portal

We encourage customers to utilize the MHS Genesis Patient Portal to access all their medical record information after April 2021.  To register, you need to have a DoD Self-Service Logon (Level 2) account.
Get your medical record information at your fingertips by taking advantage of the Patient Portal. Some records available in the Patient Portal include but not limited to:

  • Lab Results

  • Radiology Reports

  • Clinical Notes & Documents

  • Immunization Records

  • Medication Lists

  • Appointment Lists

  • COVID-19 Results
     

Medical Records Requests

Effective November 20, 2023, Irwin Army Community Hospital has implemented a new method for delivering completed medical record requests to our valued customers. We are pleased to introduce the use of the DoD Secure Access File Exchange (DoD SAFE), a secure and encrypted platform. Our primary goal with this new approach is to streamline the process, reducing wait times and eliminating unnecessary steps that might cause delays.
 

To Start Your Request for Medical Records: 


Each patient is entitled to receive one complimentary copy of their medical records. However, requests for duplicate copies will incur a charge of $18.97, payable upon request.

Upon submission, you will receive a text message notification indicating when to check your email for the DoD SAFE link to retrieve your records. Kindly check your junk mail folder for a DoD SAFE link to retrieve your records. Note that charges for this text notification may apply depending on your phone service provider.  

At IACH, we endeavor to fulfill requests in a timely manner. Please be advised that routine requests typically require up to thirty (30) working days for completion. We encourage you to utilize the Patient Portal whenever possible for expedited service.


Active Duty (AD) Members

AD members may request a copy of their medical record by completing and digitally signing the DD Form 2870. Once completed and signed, you may send your request to dha.riley.irwin-ach.mbx.iach-ft-riley-correspondence-roi@health.mil

Family Members

Family members may request a copy of their medical record by completing and hand signing the DD Form 2870. You may be asked to provide a copy of a photo ID with signature such as a driver’s license or state ID, so your signature can be validated if our facility does not have your signature on file. To avoid delays, you are encouraged to include your photo ID such as a driver’s license at the time of your request. A military dependent card is insufficient.

Stepparents cannot request copies of stepchildren without parent consent or medical power of attorney regardless of if the stepparent is the sponsor of the stepchild. 

Veterans

If you are a veteran and continuing to receive care at IACH, we will be able to provide you with a copy of your medical record available in our electronic heath record. Reference Archived Records for records during AD service.
 

Behavioral Health Records

If you are requesting Behavioral Health (BH) records from another MTF prior to April 2021, you must request them directly from the originating MTF. Visit TRICARE and enter the MTF name in the search box. Follow the MTF’s instructions posted on their site.

If the BH records are after April 2021, IACH will be able to provide them to you in most cases regardless of originating MTF.

BH records related to Family Advocacy Program (FAP), or Substance Use Disorder Clinical Care (SUDCC) will require additional forms for release and may require an additional 30 working days to process.

 

Medical Records Request for Legal, Insurance or Background Checks

All requests to include but not limited to legal, insurance or background checks will be processed by Medical Records. Please fax written request to (630) 570-5781 or email to dha.riley.irwin-ach.mbx.iach-ft-riley-correspondence-roi@health.mil

All medical records will be sent via DoD Safe. Please include an email address on the written request. Requestor will have seven (7) days to retrieve the file(s).

You may be asked to furnish a form of ID to validate your identity as the requestor or a photo ID of the patient to validate their signature if we do not have it on record. To avoid delays, include ID(s) at the time of your request.

We may charge for copy services when appropriate. Payment is due upon receipt of invoice from non-Federal customers. All payments are made via Pay.gov. Records will typically be available within 24-48 hours of payment.

 

In/Out-Processing Medical Records

All In/Out Processing is completed at the medical records window. All AD must report to Medical Records with a copy of their Orders and/or the DA Form 877 from the losing Military Treatment Facility (MTF). Please reference our office hours.
 

Preparing to Retire or Separate

AD members preparing to retire or separate should obtain copies of all medical records at least six (6) months prior to their final out-processing date.

AD members should return to Medical Records to obtain updates of medical documents generated after their records were initially copied. By doing so, members can ensure they receive a comprehensive copy of their AD medical record following their retirement or separation.

 

Archived Records

IACH maintains outpatient records for a period of two (2) years after the patient’s last medical encounters (excluding AD and their family members). Inpatient records are maintained for two (2) years after the date of discharge. After such periods, the records are retired.
