Access Your Medical Records
At Irwin Army Community Hospital, we make it easy for you to access your medical records. Here’s how:
MHS Genesis Patient Portal
We encourage you to use the MHS Genesis Patient Portal to access your medical records. To register, you’ll need a DoD Self-Service Logon (Level 2) account.

Using the portal is the fastest way to access your records.
Requesting Medical Records
If you need a copy of your medical records, we use the DoD Secure Access File Exchange (DoD SAFE), a secure and encrypted platform, to deliver them.
How to Request Records:
- Complete the DD Form 2870.
- Email the form to: dha.riley.irwin-ach.mbx.iach-ft-riley-correspondence-roi@health.mil.
Important Notes:
- Cost: Your first copy is free. Duplicate copies cost $18.97, payable upon request.
- Timeline: Routine requests take up to 30 working days.
- Notifications: You’ll receive a text and email with a DoD SAFE link to retrieve your records. Check your junk folder if you don’t see it. Link is active for only 7 days. If the link expired, you may be asked to submit a new request.
Special Cases
Active-Duty Service Members
Submit a digitally signed DD Form 2870 to the email above.
Family Members
Submit a hand-signed DD Form 2870. Include a photo ID (e.g., driver’s license) to validate your signature.
Veterans
For records during active duty prior to 2015, visit the National Archives. For current care records, use the process above.
Behavioral Health Records
Behavioral Health (BH) records may require an extra 30 days to process. Family Advocacy Program (FAP) or Substance Use Disorder Clinical Care (SUDCC) records may require additional forms and 30 extra days to process.
Legal, Insurance or Background Check Requests
Fax or email your written request to:
Include an email address for DoD SAFE delivery. Records are available for 7 days after sending.
Payment may be required for non-Federal requests via Pay.gov.
In/Out-Processing Medical Records
Active Duty: Bring your Orders or DA Form 877 to the Medical Records window.
Retiring/Separating: Obtain copies of your medical records 6 months before your final out-processing date. Return for updates before leaving.
Archived Records
Outpatient records are kept for 2 years after your last visit (excluding AD and family members).
Inpatient records are kept for 2 years after discharge.
